Be sure to include your first and last name(s), last 4 digits of your social security number and current mailing address.

To speed up the review process you can provide any documentation that you would like us to consider. You may be asked to provide proof that you applied with SCHA. 

You will receive a written response from our office within 60-90 days.

CORRECTING INFORMATION ON YOUR APPLICATION

We are only processing requests to change the following information:

*Date of birth of head of household only

*Social Security Number of head of household only

*Address

To request to update this information do the following:

Send an email to schainfo@cfl.rr.com, or fax your request to 407-359-2576. Please include your full name and last 4 digits of your social.

Be advised only the above information will be changed/updated. Any other changes to your application can be made once your application is pulled to receive assistance. You will receive a written  response within 60-90 days.
